    DOCUMENTS REQUIRED FOR FSSAI LICENSE REGISTRATION

    General documents required for basic FSSAI registration.

    1. Authorized person address proof such as Aadhar card, driving license, PAN card
    2. Passport size photo
    3. Business name and address
    4. Form IX nomination of a person as per FSS rules
    5. Details of directors/ proprietors and executive members
    6. PAN and bank details
    7. Nature of business details
    8. NOC from the municipality or panchayat
    9. Proof of the premise ownership or rental agreement
    10. NOC from the owner of the rented property ( if rented )

    Document required for FSSAI State license other than general documents.

    1. Form-B Duly completed and signed by the proprietors, partners, or authorized signatory
    2. Copy of certificate obtained under COOP-ACT, 1861/multistate COOP-ACT, 2002 in case of cooperatives
    3. Partnership deed/ memorandum and article of association toward the constitution of the firm
    4. Layout plan of the processing unit showing the dimensions in square meter/ meter ( mandatory for manufacturing and processing units only )
    5. Authorized letter with name and full details of the responsible person nominated by the manufacturer and alternative responsible person
    6. List of directors, proprietors, and executive members of society or trust with full detail
    7. Name and list of all the equipment and machines along with numbers installed capacity and horsepower
    8. Chemical and bacteriological analysis report in case of water used as an ingredient.
    9. IE code document issued by DGPT
    10. Recall plan wherever applicable
    11. Certificate provided by the ministry of tourism
    12. Form IX
    13. Declaration in the case of Haryana and Delhi
    14. Food safety and management plan

    Documents are required for FSSAI central license. Form-B Duly completed and signed by the proprietors, partners, or authorized signatory

    1. Form IX – nomination of a person by the company along with the board resolution
    2. NOC and a copy of the license from the manufacturer are mandatory for repackers only
    3. Ministry of Commerce certificate for 100% EOU
    4. NOC/PA Document issued by FSSAI

    Renewal Of FSSAI Registration

    FSSAI registration renewal is as important as getting a new registration. The license is issued for the validity of 1-5 years. Hence all the food business operators must file for FSSAI renewal 90 days before the due date.
